For ETC Players:
If you have any questions while filling out these forms, please give us a call and we will assist you in doing it .You will need to provide one registration form, one medical release and one rules and regulations form for each player.

For GPTCA Coaches:
- Read Course rules and instruction.
- Complete and submit GPTCA Registration Form Below
- Pay Required fees prior to registration deadline.
